Computer Voucher Program

Connections offers a program in which registered consumers in our seven country service region can receive a new Windows or Chrome laptop computer for personal use.

Our Computer Voucher Program will allow the user to connect to the Internet, and use technology to order food, medication, and supplies for home delivery, along with connecting to others through support groups.

Program funding through the federal CARES Act allows for a maximum of $350 to be applied to any computer order. There are a limited number of computers available through this program, so persons who are interested in applying should contact their Connections Independent Living Specialist. New recipients are selected monthly through this program.

Computer orders are fulfilled by Greeley Computer Services. Recipients are responsible for any applicable local and state taxes. Complimentary home delivery of the completed computer is available upon request.

Contact Connections at (970) 352-8682 to learn more about this program, or to be added to our interest list.

SSI/SSDI Evaluation Program

Connections offers assistance in screening for eligibility to Social Security’s Supplemental Security Income (SSI), and Social Security Disability Insurance (SSDI). This free evaluation for a person who lives with a disability within the Connections service region is by a trained SSI/SSDI benefit specialist. Our team can help guide the candidate through the qualification process. There is no obligation through participation in this program, and it is confidential. Contact Connections by telephone at (970) 352-8682 to request additional information, or to schedule your evaluation session.

COVID-19 Testing/Vaccination Guidance

Connections offers free guidance to consumers regarding the testing and vaccination processes related to COVID-19. Team members adhere to strict confidentiality agreements, and all staff undertake background checks to assure safety of personal information. Team members from our Greeley-based nonprofit can immediately offer assistance in registering and scheduling testing for COVID-19, or in receiving the vaccination according to state policy. Free transportation to and from vaccine appointments can also be arranged.

Individuals who have an interest in receiving assistance are invited to call (970) 352-8682 weekdays during standard business hours to arrange a consultation. Like most services at Connections, this appointment is provided at no cost to qualifying service area residents who live with a disability.

Connections Cares Program

Launched in October 2020, and resuming onsite services in March 2021, our Connections Cares program provides in-person assistance at our Greeley headquarters.

Each Friday from 11 a.m. until 1 p.m.,  a team of independent living specialists will be at our office to provide assistance and guidance to persons who live with a disabling condition within our seven-county service region. Knowledgeable representatives can provide assistance and guidance regarding independent living, housing options, SSI/SSDI eligibility, and COVID-19 testing and vaccinations.

Persons who are temporarily experiencing homelessness are welcome.

The program also offers at no cost nutritious pantry food boxes to supplement home supplies, along with personal protective equipment for safety during the COVID-19 pandemic.

Appointments for services are recommended.

This program helps persons who live with disabling conditions to meet some of their urgent needs, along with providing the information necessary to make long-lasting changes in their lives.

The highest status of care and safety for all has been taken into account. Masks and health check-in are mandatory prior to entering our offices. Social distancing is required.

Heavy snow, cold temperatures, or other inclement weather may cancel a session. Call us at (970) 352-8682 if the weather is poor or uncertain for confirmation that our Connections Cares team will be on site.
