Launched in October 2020, our Connections Cares program provided scheduled in-person assistance at our Greeley office until the program conclusion on May 28, 2021.
Through this program, a team of independent living specialists provided assistance and guidance to persons who live with a disabling condition within our seven-county service region. Knowledgeable representatives provided assistance and guidance regarding independent living, housing options, SSI/SSDI eligibility, and COVID-19 vaccinations.
Persons who are temporarily experiencing homelessness were welcome.
During the winter season, the program also offered at no cost nutritious pantry food boxes to supplement home supplies.
This program helped persons who live with disabling conditions to meet some of their critical needs, along with providing the information necessary to make long-lasting changes in their lives.
